Request Form for Access to Administrative Information
Any person, without having to state an interest, has the right to access administrative documents, which includes the rights to consult, reproduce, and obtain information about their existence and content. However, there may be restrictions on this right.
Access to administrative documents must be requested in writing, through an application that contains the essential elements for identifying the applicant, namely name, personal or corporate identification details, contact information, and signature.
For this purpose, ERS provides the following form.
Please note: this form is not intended for requests for clarification regarding users’ rights and duties; ERS procedures in handling cases; nor for specific information requests concerning the registration and licensing of providers. These requests should be made, respectively, through the information request form regarding users’ rights or via the email address registos@ers.pt for matters related to registration and licensing.
The form provided on this page must be used to request access to documents contained in case files and to obtain information about the status of specific case(s).
